What to Bring

If you have made an appointment for your initial consultation, please bring in the following:
  • A chronology of relevant events 
  • Written communication, including e-mails, between you and your employer, concerning your claim or your job performance, or from co-workers regarding your claims
  • Contact information for witnesses
  • Documentation of any complaints about your concerns
  • Copies of disciplinary action taken against you
  • Your performance evaluations, documents relating to your job performance
  • Personal logs or recordings
  • If you have filed with an administrative agency, such as the New York State Division of Human Rights, a copy of the file obtained from the agency
  • Your employment manual and other documents showing employer’s policies
  • Your resume 
  • Medical records relating to a disability or psychological effects of the employer’s conduct
  • Your W-2's and last pay stubs
  • Benefit documents, such as health and pension
  • Employment Contracts,Union Contracts
  • If you have a wage complaint, your timesheets, documentation of your hours worked each week and pay each week
  • Documentation of your job search if you have been fired and have not gotten another job 
  • Any other documents which you think will be helpful in evaluating your claims
